SNAP Document Checklist

Documents you may need for your SNAP application

SNAP stands for the Supplemental Nutrition Assistance Program (formerly known as the food stamp program). SNAP offers nutrition assistance to millions of eligible individuals and families and provides economic benefits to your local community.

 

When applying for SNAP you are required to submit supporting documentation of household income and certain expenses. You may need the following:

 

1. PROOF OF IDENTITY

☐ Birth Certificate

☐ Driver’s License/ State Photo ID

☐ Social Security Card

 

2. PROOF OF SHELTER EXPENSES

☐ Utility Bills – like electric, gas, water

☐ Rental Agreement (lease) or Mortgage Statement that shows your address

☐ Most Recent Property Taxes

☐ Insurance Statement (Homeowner’s or Renter’s)

☐ Shelter Verification Form

 

3. MEDICAL EXPENSES

(Elderly (60+) or Disabled households only)

☐ Medicare Part B &D Premiums

☐ Billing Statements/ Unpaid Medical Bills

☐ Itemized Pharmacy Print-Out for 90 days

 

4. PROOF OF EARNED INCOME

☐ Paystubs for the last 30 days

– current and consecutive.

☐ Statement from employer as to gross wages

☐ Income tax forms

☐ Self- Employment Worksheet

 

5. PROOF OF UNEARNED INCOME

☐ Bank Statements showing direct deposit

☐ Agency Letter showing money received, ex Social Security, SSI, VA benefits, Child Support, Alimony, Unemployment, and Retirement
